Police officials said the assault took place on December 3 in the Sunni town of Tarmiyah, some 50 kilometers north of the capital.
There were several police officers among the dead.
Reports say one attacker wearing an explosives belt blew himself up while others shot at security guards and fired mortars.
No group immediately claimed responsibility for the assault.
Meanwhile, in the northern city of Tikrit, two police officers were killed in an attack on a police administrative building. A car bomb was followed by clashes between militants and security forces.
Sunni Muslim insurgents regularly hit targets linked to the Shi'ite-led government and security forces.
